Parental consent form

For guests under 18, we need this form signed by a parent (legal guardian). Print it, fill it in together, and hand it in at the front desk before the first visit. A pass bought online cannot be used until it has been handed in.

You can also save it as a PDF from the print dialog. The rules are in the “Companions and children” section of the House rules. The form is also available in Hungarian.

Consent form for a guest under 18 to train at Forma 1 Fitnesz (F1F Kft.)

I, the undersigned name of the legal guardian , as the legal guardian of name (born: YYYY.MM.DD. ), declare that:

  1. I consent to the minor using the services of Forma 1 Fitnesz within the limits set out in the House rules (under 16, only in personal training sessions, under the supervision of a trainer; between 16 and 18, also on their own; no group classes under 18);
  2. if the minor bought the pass: I approve the contract concluded by the minor for the name of the pass pass bought on YYYY.MM.DD. ;
  3. I have read the GTC and the House rules.
